About Budecort Rotacap 400 mcg

Budecort Rotacap 400 mcg is a prescription dry powder capsule containing 400 mcg of budesonide, made by Cipla Limited, used as maintenance treatment for moderate to severe persistent asthma in adults. It is the strongest Budecort Rotacap, letting people on higher inhaled steroid doses take one capsule instead of two or four lower-strength ones.

How does high-dose budesonide help?

In more severe asthma, airway inflammation is more intense and harder to settle. A higher inhaled dose of budesonide can bring it under control and may reduce the need for courses of steroid tablets, which carry far more side effects. Like all preventers, it works gradually and does not relieve an attack.

Who is the 400 mcg strength for?

  • Adults whose prescribed dose is 400 mcg or more per inhalation.
  • People stepping up after poor control on lower doses, usually while a specialist reviews them.
  • Patients reducing steroid tablets under close supervision.

Children seldom need this strength.

How to take it

The usual adult range is 200–800 mcg twice daily; with this capsule that means one or two capsules morning and evening, up to a maximum of 1,600 mcg a day. Load one capsule at a time into the Rotahaler or Revolizer, breathe out away from the device, then inhale rapidly and deeply and hold your breath. Confirm the shell is empty before loading the next capsule. Rinse and spit afterward, since high doses make thrush and hoarseness more likely.

Monitoring at higher doses

Because more budesonide is absorbed at high doses, doctors keep an eye on:

  • Adrenal function: long-term high doses can reduce the body’s own cortisol, so extra steroid cover may be needed during severe illness, injury or surgery, and some people carry a steroid card.
  • Bones: discuss bone health if you have other risks for osteoporosis.
  • Eyes: regular eye checks for cataracts and glaucoma, and prompt review of blurred vision.
  • Blood sugar: it can rise slightly in people with diabetes.
  • Dose: once asthma has been stable for about three months, the aim is to step down.

What are the side effects?

Voice changes, oral thrush, sore throat and cough are the ones most often reported. Bruising, skin thinning, restlessness and sleep problems occur less often. Get urgent help for sudden breathlessness after a dose, signs of an allergic reaction, severe tiredness with dizziness, vomiting or fainting (possible adrenal insufficiency), or an asthma attack that your reliever does not control.

Interactions and cautions

Strong CYP3A4 inhibitors, including ritonavir- or cobicistat-boosted HIV medicines, itraconazole, ketoconazole and clarithromycin, can raise blood levels enough to cause Cushing’s syndrome with budesonide, so your doctor may choose another option. Tell your doctor about tuberculosis, liver disease, infections or eye disease. Budesonide can be continued in pregnancy and breastfeeding when needed, with your doctor using the lowest dose that controls asthma.

Pack sizes and storage

Packs of 30, 60 or 90 capsules, from $7. Store in a dry place below 86°F (30°C), leaving capsules in their foil strip until use.

Frequently asked questions

Is 400 mcg a high dose?

One capsule twice daily (800 mcg a day) counts as a medium adult dose; two capsules twice daily is a high dose.

Could a combination inhaler suit me better?

Guidelines often prefer adding a long-acting bronchodilator to raising the steroid alone, for example Foracort Rotacaps 400 mcg. Lower doses are possible with Budecort Rotacap 200 mcg.

What if I find capsules hard to use?

A nebulizer form, Budecort Respules 1 mg, is available, and the respiratory range lists other devices.

General information, not a treatment plan. Budecort Rotacap 400 mcg is a prescription medicine for use under regular medical review.
