Strongyloidiasis: why treatment matters
Strongyloides stercoralis is a threadworm found in warm regions worldwide, including rural parts of the southeastern United States. It can persist in the gut for decades by reinfecting its host, often causing only vague abdominal pain, diarrhea, cough or an itchy, creeping rash. The danger comes when immunity falls: corticosteroids, cancer treatment, organ transplants or HTLV-1 infection can trigger hyperinfection, in which larvae spread throughout the body, and this can be fatal. Ivermectin is the first-choice treatment, and screening is often advised before long-term steroid therapy in people from endemic areas.
How does Ivermectol work?
By binding to chloride channels that only invertebrates use in their nerves and muscles, ivermectin paralyzes the threadworms living in the intestine. Blood levels peak about four hours after a dose, and the drug is eliminated over the following days, almost entirely in the feces.
How to take Ivermectol 12 mg
- The labeled strongyloidiasis dose is about 200 micrograms per kilogram once; one 12 mg tablet suits an adult of roughly 132 lb (60 kg).
- Take it on an empty stomach with a full glass of water.
- Some doctors give a second dose about two weeks later, and people with weakened immunity may need longer specialist-led courses.
- Stool tests or blood antibody tests are repeated afterward to confirm the infection has cleared.
What are the side effects?
Side effects after treating threadworm are usually mild and brief: diarrhea, nausea, loss of appetite, dizziness, sleepiness or itching. Blood tests occasionally show a temporary rise in liver enzymes or a dip in white cells. Go to an emergency department for confusion, trouble staying awake, poor coordination, seizures, a severe rash or difficulty breathing.
Who should not take it?
- Anyone allergic to ivermectin.
- Pregnant women and children under 33 lb (15 kg), unless a specialist advises otherwise; breastfeeding mothers should ask first.
- People who may carry Loa loa, until they have been tested.
- Those on warfarin, who may need an INR check; also mention sedatives and every other medicine you use.
