How does tadalafil 20 mg work?
Tadalafil is a PDE5 inhibitor. By stopping the breakdown of cGMP in erectile tissue, it keeps the blood vessels of the penis relaxed for longer once arousal has started, so blood flow rises and firmness holds. A higher dose does not make an erection bigger than normal; it simply improves the odds of a dependable response in men whose ED is moderate or severe.
Is 20 mg the right strength?
In clinical trials, more men reported successful intercourse on 20 mg than on 10 mg, although headache and back pain were also reported more often. A doctor may choose 20 mg when 10 mg has given only a partial response, or for ED linked to diabetes or pelvic surgery. This strength is not for everyone: men with reduced kidney or liver function, and those taking strong CYP3A4 inhibitors such as ketoconazole, itraconazole or ritonavir, have lower dose limits that a 20 mg tablet would exceed.
How to take Megalis 20 mg
Swallow one tablet whole with a glass of water before planned sexual activity. It usually begins to work within 30 to 60 minutes, occasionally up to two hours, and can stay active for up to a day and a half. Do not take a second tablet in the same 24 hours, even if the first seems slow, and never combine it with any other tadalafil or sildenafil product.
Important safety warnings
- Nitrates: never take Megalis with nitroglycerin, isosorbide or “poppers”. If a nitrate is needed in an emergency, tell the medical team when you took tadalafil; at least 48 hours should normally pass after your last dose.
- Riociguat: this combination is contraindicated.
- Alpha-blockers and blood pressure drugs: together they can cause light-headedness or fainting, so a lower starting dose may be chosen.
- Alcohol: five or more drinks with tadalafil can cause a drop in blood pressure on standing.
- Heart health: ask a doctor whether your heart is fit enough for sexual activity before starting.
What are the side effects?
Headache, heartburn, facial flushing, stuffy nose, back pain and aching legs are the most common, and they tend to be more noticeable at 20 mg. Seek urgent care for a painful or prolonged erection over 4 hours, sudden vision loss, a sudden decrease in hearing, fainting, or chest pain.
Other uses of tadalafil 20 mg
Specialists also use tadalafil 20 mg tablets for pulmonary arterial hypertension, at 40 mg once daily. That is a separate condition needing specialist monitoring, and Megalis should not be used for it without that care.

Ask your pharmacist first; a lower strength such as Megalis 10 mg gives a more accurate dose than splitting.
