About Seroflo Rotacaps 250 mcg

Seroflo Rotacaps 250 mcg are prescription inhalation capsules containing salmeterol 50 mcg and fluticasone propionate 250 mcg, made by Cipla Limited, used for twice-daily maintenance treatment of moderate asthma and of chronic obstructive pulmonary disease (COPD). The powder is breathed in through a Rotahaler or Revolizer capsule device. This medium-strength combination corresponds to the 250/50 dry-powder dose, which is the strength US labeling uses for COPD.

How does Seroflo Rotacaps 250 mcg work?

In asthma and COPD, the airways are both inflamed and narrowed. Fluticasone propionate is an inhaled corticosteroid that reduces inflammation in the bronchial lining; in COPD it is mainly used to lower the number of flare-ups in people who have them often. Salmeterol is a long-acting bronchodilator that holds the airways open for roughly 12 hours, easing breathlessness during daily activities. Neither ingredient provides instant relief.

Who is it for?

  • Asthma, age 12 and over: when asthma is not controlled on a low-strength combination or on a steroid inhaler alone.
  • COPD: for adults with chronic bronchitis or emphysema who keep having flare-ups despite regular bronchodilators. Many specialists now pair an inhaled steroid with two long-acting bronchodilators, so you may also be prescribed a separate LAMA inhaler such as tiotropium.

How to take Seroflo Rotacaps 250 mcg

Inhale the contents of one capsule twice a day, roughly 12 hours apart. Do not use more than two capsules in 24 hours, as salmeterol’s labeled maximum is 50 mcg twice daily. Load a capsule into a dry device, breathe out away from the mouthpiece, then breathe in sharply and deeply so you hear the capsule rattle. Hold your breath for several seconds, check the shell is empty, and repeat if needed. Rinse with water and spit after every dose to protect your mouth and throat. A bottle of 30 covers 15 days of treatment.

COPD: what to watch for

People with COPD who use inhaled steroids are more likely to develop pneumonia. Its signs can resemble a flare-up, so contact your doctor promptly about fever, chills, sharper chest pain, more breathlessness, or thicker or discolored sputum. Stay up to date with the vaccines your doctor recommends, keep using any other COPD inhalers as prescribed, and if you smoke, stopping is the single most effective way to slow lung damage.

What are the side effects?

  • Common: hoarseness, sore throat, oral thrush, headache, muscle and joint pain.
  • From salmeterol: tremor, palpitations, a faster heart rate, and occasionally low potassium or higher blood sugar.
  • With long-term use: bruising, reduced bone density, cataracts or glaucoma; ask about regular eye checks.
  • Get urgent help for a sudden worsening of breathing after a dose, chest pain, an irregular heartbeat, or swelling of the face, lips or throat.

Who should take care?

Your doctor needs to know about heart disease, high blood pressure, seizures, diabetes, an overactive thyroid, osteoporosis, eye disease and any history of tuberculosis. Strong CYP3A4 inhibitors such as ritonavir, cobicistat, ketoconazole and clarithromycin are generally avoided with this inhaler. Non-selective beta-blockers, diuretics, MAO inhibitors and tricyclic antidepressants need review. Do not combine it with another LABA such as formoterol. The capsules contain lactose. In pregnancy and breastfeeding, the doctor will weigh the benefits of good breathing control.

Pack sizes and storage

Packs of 30, 60 or 90 capsules, from $10.20. Store below 77°F (25°C) and away from moisture; keep the container tightly closed. A Rotahaler or Revolizer is needed separately if you do not already have one.
