About Seroflo Multihaler
Seroflo Multihaler is a prescription multi-dose dry powder inhaler containing salmeterol 50 mcg plus fluticasone propionate (100, 250 or 500 mcg, depending on the strength) in each inhalation, made by Cipla Limited, used for twice-daily maintenance treatment of asthma and, at the higher strengths, chronic obstructive pulmonary disease (COPD). Each device holds 60 pre-measured doses, a full month of treatment at one inhalation morning and evening. It works on the same principle as the Diskus or Accuhaler devices used for Advair and Seretide.
Which strength do you need?
| Strength (fluticasone/salmeterol) | Typical use |
|---|---|
| 100/50 | Asthma needing a low steroid dose, adults and children 4 and over |
| 250/50 | Moderate asthma from age 12; the strength US labeling uses for COPD |
| 500/50 | Severe asthma from age 12; the COPD strength in UK and European labeling |
The product listing does not state a strength, so check that the label on the device you receive matches your prescription before using it.
How does Seroflo Multihaler work?
The device holds 60 measured doses of powder and sets up one at a time when you load it; your own inhalation then carries the powder into the lungs. Fluticasone calms airway inflammation, and salmeterol keeps the airways open for around 12 hours. Because the device is breath-activated, there is no propellant and no need to coordinate pressing and breathing, but you need a steady, deep breath to draw the powder out.
How to use Seroflo Multihaler
- Hold the device level, open it and load one dose as the leaflet shows; do not open or load it again until you are ready, as this can waste doses.
- Breathe out fully, turning your head away so you do not blow into the mouthpiece.
- Seal your lips around the mouthpiece and breathe in quickly and deeply through your mouth, not your nose.
- Remove it, hold your breath for about 10 seconds, then breathe out slowly.
- Close the device, then rinse your mouth with water, gargle and spit.
Keep the device dry, never wash it, and wipe the mouthpiece with a dry tissue. If your device has a dose counter, order the next one before it reaches zero.
How much to use
The usual dose is one inhalation twice daily, about 12 hours apart, and never more than two inhalations a day, because the salmeterol dose must not exceed 50 mcg twice daily. Do not use it to relieve a sudden attack; a fast-acting inhaler such as Asthalin Inhaler is needed for that.
What are the side effects?
Hoarse voice, throat irritation, thrush in the mouth, headache and muscle aches are the most common. People with COPD who use inhaled corticosteroids have a higher risk of pneumonia, so report fever, chills, increased cough or a change in the color of your phlegm. Long-term use, especially at higher doses, can thin the bones, raise the risk of cataracts and glaucoma, and in rare cases suppress the adrenal glands. Seek urgent care for worsening wheeze straight after a dose, chest pain, a very fast heartbeat or swelling of the face and throat.
Cautions and interactions
Never add another long-acting bronchodilator of the same class, and do not stop treatment abruptly. Ritonavir, cobicistat and azole antifungals such as ketoconazole can push fluticasone levels high enough to cause Cushing’s-type effects. Beta-blockers, diuretics, MAO inhibitors and tricyclic antidepressants also need review. The powder contains lactose, so mention a severe milk-protein allergy. Tell your doctor about heart disease, diabetes, osteoporosis, thyroid disease or past tuberculosis.
