About Penegra 25 mg

Penegra 25 mg is a prescription tablet containing 25 mg of sildenafil citrate, made by Zydus Cadila (now Zydus Lifesciences), used to treat erectile dysfunction in men who need a lower dose. It is the smallest standard strength of sildenafil for ED. Penegra is sold in India and is not a licensed product in the UK, EU or US.

Who is Penegra 25 mg for?

Sildenafil labeling suggests a 25 mg starting dose in several situations:

  • men over 65 (advised in US labeling; UK labeling does not require a reduction for age alone);
  • mild to moderate liver impairment, such as cirrhosis;
  • severe kidney impairment (creatinine clearance below 30 ml/min);
  • men taking strong CYP3A4 inhibitors such as ketoconazole, itraconazole or clarithromycin, or erythromycin; with ritonavir, US labeling allows no more than one 25 mg dose in 48 hours and UK labeling advises against the combination;
  • men who are stable on an alpha-blocker, such as doxazosin or tamsulosin, because of the risk of low blood pressure on standing.

It also suits men who find 50 mg effective but get headaches or flushing, and want the smallest dose that still works.

How does it work?

Sildenafil blocks PDE5, an enzyme that breaks down cGMP in the penis. More cGMP means relaxed blood vessels and better blood flow when you are aroused. Even at 25 mg the mechanism is identical; the lower dose simply produces lower blood levels, which matters when the body clears the drug more slowly.

How to take Penegra 25 mg

Take one tablet with water about an hour before sex, with or without food, bearing in mind that a fatty meal may delay it. It acts for around four to five hours, and one dose in 24 hours is the maximum. If 25 mg is well tolerated but not effective, your doctor may raise the dose to Penegra 50 mg; do not simply take extra tablets on your own.

Important safety information

Penegra must not be taken with nitrates, including nitroglycerin (glyceryl trinitrate), isosorbide mononitrate or dinitrate and nicorandil, with amyl nitrite (“poppers”), or with riociguat. It is not for men with severe heart disease, a recent heart attack or stroke, low blood pressure, severe liver impairment or hereditary retinal conditions. A lower dose does not make any of these combinations safe. Stop sexual activity and seek help if chest pain develops, and tell the paramedics you have taken sildenafil. Never combine it with another sildenafil form or a different ED tablet.

What are the side effects?

Side effects are dose-related, so they tend to be milder at 25 mg, but they can still happen: headache, flushing, indigestion, a blocked nose, dizziness and short-lived visual changes. Older men and those on blood pressure medicines should stand up slowly if they feel light-headed. Seek urgent care for an erection lasting more than 4 hours or sudden loss of vision or hearing.
