Why tadalafil can be taken two ways

Like sildenafil (Viagra), tadalafil is a PDE5 inhibitor. It blocks the enzyme that breaks down cGMP, the chemical messenger that relaxes blood vessels in the penis during arousal, so an erection is easier to get and keep. It does not cause an erection on its own; sexual stimulation is still needed.

How daily tadalafil works

With the daily plan you take one low-dose tablet at about the same time each day, with or without food. Blood levels rise over the first few days and reach a steady state after around five days, so the full benefit is not instant. After that, you do not need to time anything around sex.

  • Starting dose: the US label starts daily use for ED at 2.5 mg, which can be raised to 5 mg. For an enlarged prostate, or ED and BPH together, the dose is 5 mg.
  • Missed dose: take it when you remember the same day, but never take two tablets in one day to catch up.
  • Give it time: judge the plan after a few weeks of regular use rather than the first couple of days.

For men who have both ED and BPH, a single daily tablet can help with both. Improvement in urinary symptoms is usually noticeable within a few weeks.

Urinary symptoms can have other causes, including infection and prostate cancer, so have them checked by a doctor before treating them yourself. Price is often the deciding factor. Daily dosing needs about 30 tablets a month. As-needed use twice a week needs about 8 or 9. Get medical help straight away for an erection lasting more than 4 hours, sudden loss of vision in one or both eyes, sudden hearing loss or ringing in the ears, or chest pain, dizziness or fainting during sex.

Who should not take tadalafil

  • Anyone using nitrates for chest pain, such as nitroglycerin sprays or tablets and isosorbide mononitrate or dinitrate, or recreational “poppers” (amyl nitrite). The combination can cause a dangerous drop in blood pressure. If you develop chest pain after taking tadalafil, tell the emergency team when your last dose was, because nitrates should not be given within 48 hours of it.
  • Anyone taking riociguat for pulmonary hypertension.
  • Men whose heart is not healthy enough for sexual activity, or who have had a recent heart attack or stroke, or have very low or uncontrolled high blood pressure.
  • People with severe liver disease, for whom tadalafil is not recommended, and people with severe kidney disease, who should not use daily dosing and need a much lower as-needed limit.

Strong CYP3A4 inhibitors, including ketoconazole, itraconazole, clarithromycin and HIV medicines such as ritonavir, raise tadalafil levels; your doctor will lower the maximum dose (daily dosing is capped at 2.5 mg). Heavy drinking, around five or more drinks, increases the risk of dizziness and fainting. Never combine tadalafil with another ED tablet.

Switching between plans

Many men try one plan first and switch later, for example moving to daily dosing once a relationship becomes more regular, or moving back to as-needed to cut costs. Switching is straightforward, but agree the new dose with your doctor and remember the golden rule: one dose of tadalafil in any 24 hours. Do not add an as-needed tablet on top of your daily one.
